Your first visit– Includes a consultation visit with an expert consultant. At that time you will be required to provide us with contact information of your previous physicians so that we can request your previous medical records or you may bring in any medical records you may have for us to review and make sure that you have sufficient and proper documentation. The appointment will also provide you with information about local dispensaries, products available to you, and help answer any questions you may have. Once the records are received into our office, we will then review them to see if you are qualified based on your medical conditions. If qualified, you will have a second appointment scheduled with the physician so we can create your profile in the medical marijuana registry.
Your visit also includes a consult with the physician. This appointment will include your profile being created in the registry so you can apply immediately with the state for your card. You will be given all the proper paperwork and information to apply online or through the mail. Our physicians will discuss your dosage with you and answer any questions you may have. Your recommendation will also be placed in your profile so when you receive your approval from the state, you will be able to go the dispensary to fill your order at that time.
Absolutely. Everything is confidential and all visits are protected under the privacy laws of HIPAA.
You will be required to show proof of residency for the state of Florida. This can be a Florida drivers license, state ID, voters registration card, or a utility bill verifying your name and Florida address.
Current times are running between 1-2 weeks. However, the process does go faster if patients submit their state card application online.
